Course aims and benefits

To provide an introduction to the principles of incident investigation and reporting, in order to establish causes of incidents, to help prevent their reoccurrence.

Course content

• Reasons for conducting Incident Investigation 
• Incident classification and recording
• Stages of Incident Investigation, including Fact Finding, Fact Analysis and Resolving Evidence Conflicts
• Tripod Beta Methodology
• Incident Report Preparation
• Case Study (Practice)

Outcomes

Successful candidates will be able to understand:

• How to use Tripod Beta Methodology to investigate incidents.
• The regulations and requirements for investigating and reporting incidents.
• How to use Incident Investigation tools to conduct an investigation effectively (including questioning, analysis of documentary evidence and capturing physical evidence).
• The meaning of the terms “Immediate Causes” and “Underlying Causes”, and how they are related in causing an incident.
• How Barriers and Human Factors play a role in causing incidents.
• How to present their findings in a meaningful way, so that the lessons are learned.

Entry requirements

There are no pre-requisites, but previous experience of involvement in Incident Investigation would be an advantage. However, it is not essential.

Delegates are formally assessed through the course, using the above techniques.
